The Usefulness of Hemoglobin Distribution Width in Establishing Diagnosis and Monitoring Treatment of Iron Deficiency Anemia

Abstract
Background : Because the hemoglobin distribution width(HDW) which indicates the heterogeneity of intraerythrocytic hemoglobin concentration is now available more easily with improvement of the automatic hematology analyzer, authors tried to
evaluatethe
usefulness of HDW as an important parameter of iron deficiency anemia(IDA).
Methods : We observed HDW by using Technicon H, l in 121 patients who had hemoglobin values of less than 120g/L and mean cell hemoglobin(MCH) values of less than 27pg on initial CBC test. They were diagnosed as iron deficiency anemia(IDA) in 93
cases
and as anemia of chronic disorders(ACD) in 28 cases. We reviewed hemoglobin(Hb). mean cell volume(MCV), MCH, mean cell hemoglobin concentration(MCHC), reticulocytes count, red cell distribution width(RDW) and HDW to compare two groups.
Results : In the IDA, they showed abnormal value(mean¡¾SD) in HDW : 3.8¡¾0.8g/dL. In ACD, they showed normal value(mean¡¾SD) in HDW : 2.9¡¾0.7g/dL. The differences between each group were statistically significant(p<0.05). The diagnostic cut off
level
of HDW as 3.1g/dL determined the IDA with 84.9% sensitivity and 71.4% specificity, and correctly classified percentages(CCP) of HDW combined with MCH or MCV+MCH in discriminant analysis showed over 80%. In addition, HDW was normalized most
rapidly
among
all parameters in evaluating of IDA after treatment.
Conclusions : We concluded that HDW is a very useful and important index to establish diagnosis, classify and monitor treatment of IDA. (Korean J Clin Pathol 1994 ; 14(1) : 49-56)
